If you are under 18, your parent or guardian must read and sign on your behalf.
If you are 18 or older, you may complete this yourself.
1. I release and forever discharge Identity Church, Dylan Wymer, and their affiliates, successors, assigns, officers, employees, volunteers, representatives, and agents from any and all claims, demands, causes of action, or liabilities of any kind, known or unknown, arising out of or relating to any injury, loss, or damage to person or property that may be sustained as a result of participation in this event, including but not limited to transportation provided by the leaders.
2. I agree to indemnify and hold harmless the Releasees from any and all claims, damages, liabilities, and expenses, including legal fees, arising out of or related to participation in the Retreat.
3. This waiver shall be binding upon myself and, if applicable, my child, and our respective heirs, legal representatives, and assigns. I acknowledge that I have read and understand this release and am signing it freely and voluntarily.
4. If signing on behalf of a participant under 18, I give permission for my son/daughter to attend the Jesus Weekend at Beulah Beach Camp from Friday, August 14 to Saturday, August 15, 2026. Participants will meet at Identity Church at 11:30 AM on Friday. Dylan Wymer and associate leaders will provide transportation. Estimated return time is 11:00 PM on Saturday. Parents or guardians are responsible for pickup unless alternate arrangements are approved by Dylan Wymer.
