William J. McCord Prevention Advocacy Award

Purpose: To recognize and honor an advocate for outstanding contributions to the field of prevention. Nominees for this award must display both vision and leadership, while promoting the field of prevention. They must also recognize that the prevention of alcohol, tobacco, and other drug abuse in the home, school, church, and community is a vital service

Eligibility: A nominee must be an advocate not working in the field of prevention, but has displayed outstanding advocacy leadership on an issue or issues that are vital to the field. The advocate must be a resident of South Carolina, and their contribution must personify the ideals of Jerry McCord, which are vision, leadership, and advocacy. The awarded nominee will receive a 1-year paid SCAPPA Membership.

Please submit the following documents in support of your nominee:

· A complete nomination form. (Complete all information on this nomination form) · Two (2) letters of recommendation from appropriate individuals, agencies, organizations or groups. Letters should include examples of significant professional contributions or accomplishments, and other additional comments. Letters are to be from two additional individuals. Letters of recommendation that come from the nominator will not be accepted. · Please include the original and one (1) copy for hard copy submissions. · THE DEADLINE IS November 1st.
