Through submitting this Google Form, I understand and agree that by providing my data or by clicking submit, I am voluntarily agreeing to the Privacy Notice and giving my full consent to AIESEC to collect, store, access and/or process any personal data I may provide herein, such as but not limited to 1) Name 2) Location and 3) Email address, whether manually or electronically, for the period allowed under the applicable law and regulations, for the purpose of AIESEC. I acknowledge that the collection and processing of my personal data are necessary for such purposes. I am aware of my right to be informed, to access, to object, to ensure or to block, to damages, to file a complaint, to rectify and to data portability, and I understand that there are procedures, conditions and exceptions to be complied with to exercise or invoke such rights. Rest assured that all given will only be used for the purposes stated above.

In compliance with the Data Privacy Act (DPA) of 2012, and its implementing Rules and Regulations (IRR) effective since September 9, 2016, I allow AIESEC, to store and use my data for the purpose of assessment and evaluation.

As such, I agree and authorize AIESEC to:

1. Use my personal data for the purpose of evaluation;
2. Store the online copy of my personal data;
3. Inform me of where my information will be used;
4. I understand and am assured that necessary precautions will be taken by AIESEC, to protect my personal information.
