I understand that the information in this form is only used to communicate information necessary to receive personal grooming services at Glowdega and will not be shared with any third parties.

I understand that withholding information or providing misinformation may result in contraindications and/or irritation to the skin from the personal grooming services I received at Glowdega.

I understand this is a cosmetic service with risks and potential side effects, and I voluntarily wish to have the services provided with knowledge of the potential side effects.

I understand that, for intimate services, my clothing choices which may include but are not limited to underwear, pants, shorts, skirts, dresses, workout attire, etc. and their fabrics in the 48 hours immediately following my service may impact the overall quality of the service I receive.

I understand that, for brow waxing, lamination, and tinting services experiencing some redness or mild sensitivity is normal. I also understand that an allergic reaction is possible and I have been advised, via this form, to patch test prior to my appointment if I think I may be allergic to lamination or tint products.

I understand that I cannot be waxed if I am currently: on blood thinners, sunburned; have recently used retinoids or exfoliants in the treatment area or been on Accutane in the past. I understand that sugaring may be a suitable alternative.

I understand that I cannot receive any personal grooming services if I have recently undergone surgery in the treatment area, have scar tissue, fungal/bacterial/viral infections, or if my esthetician identifies any other contraindication at the time of service. 

I understand that my failure to follow post care instructions may also lead to undesired results, complications or effects.

I acknowledge that the treatments I receive at Glowdega are completely voluntary and I release Fairy Glow Mother, LLC dba Glowdega as well as their employed skin care professionals from any liability.
