Berzerk Revival Showroom New Client Rental Application

Sounds like you're interested in renting from our showroom. We hope that we can work together. To better assess our compatibility, let's get acquainted. Please complete this 5-10 minute form so that we can ensure that we have what you're looking for.
We rent exclusively to fashion professionals and institutions. If you do not fit in any of those categories, please send an inquiry to berzerkrevival@gmail.com. Additionally, shoot us an email for celebrity styling with the client's name and event/use case. Fittings are accepted under special requests, send an email to further inquire.
Please complete every required question. Applications are reviewed within 24hours before any rental is confirmed. For last minute or same day rentals please call 07448689442.

What happens next:

We will review your application within 24 hours and email you to approve your application. Once you are approved, we will send your quote, the terms to sign and your payment details.
