Swim Station Registration Form
Swim Station (SS) is the newest premier swim school in Upstate South Carolina, dedicated to making swimming accessible and enjoyable for everyone. Our certified instructors are passionate about teaching and bring a wealth of experience to our programs. Whether you're a parent looking to introduce your child to the water or an adult eager to improve your swimming skills, we have tailored lessons for you. We offer private lessons, semi-private lessons, and small group lessons to suit your preferences and needs. To become a part of the Swim Station family, simply fill out our registration form, pay the $25 registration fee and one of our amazing instructors will reach out to you soon. Join us on the journey to become a confident and skilled swimmer!

Each SS client will be required to obtain a $25 Aquatics Membership with Crunch Fitness that includes a $59.99 annual fee at the end of 2 months; this membership will give you access to all of the Crunch Fitness amenities including childcare.

REGISTRATION FEE, SCHEDULING, CANCELATION POLICY and WAIVERS Please select the options that are best for you. We will do our best to accommodate your requested lesson times or offer an alternate time.

Cancelation Fees are as follows:
*24+hours/ notice = no fee
*Less than 24 hours up to 2-hour prior notice = 50% of lesson fee
*No-show or less than 2-hour notice = full lesson fee

NOTE: For semi-private you are responsible for organizing the group and groups must be comprised of children in the same age range and skill level. Children under 3 will not be taught in semi-private or small group swim lessons.

If you are planning on semi-private lessons, please list the name of the additional participant who will be in your lesson. EACH PARTICIPANT NEEDS TO HAVE A REGISTRATION FORM FILLED OUT.

Rates and Payment All payments to instructors are due prior to the start of your lesson and are to be paid to your instructor via Zelle, Venmo, Cash or Check. Tips are not required but always appreciated by your instructor. Rates are based on the length of the session and number of participants per session. Private lessons are defined as 1 participant, semi-private as 2 participants and small group as 3 participants. You will be charged at the lesson rate that applies to the type and length of lesson taught each day for private and semi-private. Small groups are pre-paid at registration on swimstation.org. Rates (all rates are for a 30-minute lesson) Private - 1:1 $45 Semi-private - 2:1 $60 ($30/per person) Small Group - 3:1 $100 ($25/per person for 4 lessons prepaid)

Registration Fee of $25 is required to complete this registration process

Additional Conditions
Registration form must be completed and returned for EACH participant PRIOR to first lesson. No refunds will be given for any reason. Lessons missed for personal reasons will not be rescheduled or refunded. Makeup lessons will only be held for lessons cancelled by Your Instructor and at the discretion of your instructor. Late arrivals will not have time added to the end of the lesson.

Participant Behavior Policy
     1. Participants are subject to a three-strike behavior policy.
              1. Verbal Warning
              2. Time Out
              3. Conference
    2. Any participant who becomes an impediment to the progress or safety of the class may be removed at the instructor's discretion. In this event no refund will be given.

Release of Liability
The Client expressly assumes all responsibility for the Clients participation in the activities under this Agreement. The Client agrees that he or she will not hold the property owner responsible for injuries or damages to the client or client’s property while on the owner’s property for the purposes of any activities schedule by Swim Station. It is the responsibility of Swim Station to ensure the area in use is safe and accessible for the services provided. The Client certifies that he or she is physically sound and suffering from no condition, impairment, disease, infirmity, or illness that would prevent the client's participation in the client's activities or endanger the health of others under the Agreement. The Client certifies that the client is of a legal age to enter into an Agreement or is entering this Agreement on behalf of a minor.

Photo Release
I hereby give Swim Station permission without limitation or obligation, to use photographs, film footage, or recording which may include my or my child's image or voice for the promotional or instructional purposes. Your image will not be sold to any third party for their private or public use.

Waiver Agreement
By submitting this registration form for Swim Station services, you are agreeing to all information listed above in the waiver section.
