Use this form to request a veteran psychiatric assessment with Dr Al Griskaitis.

All assessments include psychiatric diagnostic clarification and management-plan recommendations intended for implementation in primary care by the veteran’s GP.

Veterans may also need a DVA Initial Liability report to support a claim. Please indicate below whether a report is also being requested.

This is a non-urgent referral pathway. It is not for crisis care, acute risk, or emergency mental health concerns.
