I agree that, if accepted, scholarship funds will be used only for the project, materials, or program described in this application and within the approved department (visual art, vocal music, instrumental music, or theatre).

I agree that, if accepted, I will provide proof of purchases made with scholarship funds in the form of receipts, invoices, or screenshots of purchases.

I agree that, if accepted, I will provide photos documenting the project process and the completed project, and submit them to tgallaher@mineralarea.edu

I agree to complete the project within the approved timeline and understand that unused funds may need to be returned if the project is not completed.

I agree to acknowledge the Mineral Area Council on the Arts and the Future Artist Fund in any public programs, signage, or promotional materials related to this project, when applicable.

I grant MACOA permission to use submitted photos, descriptions, and outcomes of the project for reporting, marketing, grant documentation, and educational purposes.

I understand that this scholarship is a one-time award for the approved project and that future funding is not guaranteed.

I certify that all information provided in this application is accurate to the best of my knowledge.

I understand that failure to meet reporting or documentation requirements may result in the request for partial or full repayment of scholarship funds within 60 days of notice.
