This is standard protocol for screening and safety purposes. Your personal information will be kept confidential, and will be deleted after the screening is complete. Failure to comply with my screening will result in rejection of application.

Substance Use Policy

To ensure the effectiveness and safety of our sessions, you are required to be sober and free from the influence of any mind-altering substances, including drugs (prescription medication is ok), cannabis (CBD permitted with disclosure prior to day of session), and alcohol. Should you arrive for a session appearing to be under the influence, the session will be terminated immediately. You will be responsible for the full session fee, and no refund will be provided. In such an event, our professional relationship will be ended, and you will not be permitted to book future services.
