Norman Peter Johnson Outstanding Prevention Advocacy Award

Purpose: To recognize a prevention professional for outstanding contributions to the field of prevention. Nominees for this award recognize the importance of preventing alcohol, tobacco and other drug abuse in the home, school, church and community. They recognize that preventing addiction allows for an overall reduction in chronic diseases, crime, dropouts, child abuse, and other public health concerns.

Eligibility: A nominee must have at least five (5) years of experience as a prevention professional or advocate in the State of South Carolina. He or she must exhibit exemplary performance in the core areas of prevention: program coordination, education and training, community organization, professional growth and responsibility, and planning evaluation. The awarded nominee will receive a 1-year paid SCAPPA Membership.

· A complete nomination form. (Complete all information on this nomination form) · Two (2) letters of recommendation from appropriate individuals, agencies, organizations or groups. Letters should include examples of significant professional contributions or accomplishments, and other additional comments. Letters are to be from two additional individuals. Letters of recommendation that come from the nominator will not be accepted. · Please include the original and one (1) copy for hard copy submissions. · THE DEADLINE IS November 1st.
